During the investigation into the death of Jeffrey Epstein at MCC, the 2 DVR systems containing the video footage from cameras throughout MCC were seized. The DVRs were processed by OTD and the relevant video was uploaded to OPWAN/TTK for review by the case agent. Please see attached emails regarding the limitations and system failures of the DVR systems. Epstein was found dead in his cell, within the Special Housing Unit (SHU) at MCC, at approximately 6:33am on August 10, 2019. From interviews conducted as well as MCC records, it was revealed that Epstein was in the attomey conference area of MCC with his attorneys until after 4pm. So, the focus of video review was for the time period of August 9, 2019 at 4pm to August 10, 2019 at 7am. Please see attached email from AUSA pertaining to Epstein's time at MCC (MCC Timeline). Also, please see the attached spreadsheet (MCC Video Review 8-9 to 8-10 4p-7a) for a detailed description of the review of the camera within the SHU for this time period. This was sent to the AUSAs assigned to the investigation (as seen in attached email). Epstein's death was ruled a suicide via hanging by NY OCME. Once this determination was made, the investigation switched focus to the actions of the MCC Correctional Officers (COs) who were working the night/morning of August 9 -August 10. The investigation revealed that COs were to conduct rounds throughout shifts and inmate counts at certain times. The COs working in the SHU during the time that Epstein died had documented that they had conducted these required rounds and counts, but the video review revealed that they had not actually conducted all the rounds and counts. So, the review of video expanded to the available footage of the camera within the SHU to determine when COs within the SHU did or did not do the rounds and inmate counts.
