Dr. Merrell
You have an appt with Dr. Merrell at 9:30am on Thursday Oct. 9th with complete
blood work up. They request you fast from midnight until the appointment. If
you want to be tested for lyme or mercury we must let them know so they can fill
out the requisition form. Please advise.
