Subject Paper work for Dr. Kline!/He is running on time for 2pm
I was told it has been 2 years since you last saw Dr. Kline, therefore you must fill out new
paperwork...I have done as much as I can but you must sign in a few places and fill out some
personal questions. I have on my desk for you but also scanned and emailed to his assistant. I
included a copy of your insurance card.
I am told Dr. Kline is running on time for your 2pm appointment.
