Can you check with Johnny (or whoever does your dental plan) United Health
Dental says you had the plan for one day only then it was terminated-so makes me
think we are not on United Health Care but on something different. These new
cards are not good...WE need to give the dentist the correct info to claim your
cleanings this past Sat. Feb. 23, 2019
