Thanks Synopsis: On July 23, 2019, at 1:27 a.m., staff responded to a call for assistance in the Special Housing Unit (SHU). Upon arrival, the Operations Lieutenant found inmate Epstein, Jeffrey, Register No. 76318-054, lying on the floor in a fetal position. The Lieutenant called out to the inmate; however, he did not respond and she noticed red marks on his neck. The inmate was placed in hand restraints and escorted to the Suicide Cell and placed on Suicide Watch. While on watch, the inmate indicated that his cell mate had attempted to kill him and had been harassing him and he had informed his Attorney of this matter. Medical Assessment: The inmate was medically assessed at 6:30 a.m., by the Physician Assistant and his findings were as follows: He is ambulatory, oriented x 3. In no apparent distress, smiling during this clinical encounter. Alleges, that he does not know what happened. Cannot explain the marks on his neck. Responded: "I don't know". He does not want to talk of the events leading to the marks on his neck. He does not look in any distress or pain. Has a circular line of erythema at the base of the neck. Reaching 2/3 of the neck circumference, 2 inches wide, sparing the back of the neck. Has one section of this erythema in the front with marks of friction. No inflammation, no deformities, no hematomas, no lacerations, no tenderness. Patient moving his neck without any restriction. Denies having any pain or discomfort. Denies any respiratory problem. Has another small erythema on left knee about 2cm in diameter (mild). Psychologist Assessment: He was evaluated on today at 9:10 a.m., and it has been determined he will remain on Suicide Watch for further observation. Mr. Epstein arrived on July 8, 2019, and was seen by psychology services for an intake screening in SHU. He denied any psychology history or symptoms. The Chief Psychologist spoke with Central Office on July 8, 2019, and it was determined he should be placed on suicide watch and seen for a suicide risk assessment after he returns from court due to his increased suicide risk factors. Mr. Epstein was seen on July 9, 2019, for a suicide risk assessment where he denied any suicide ideation, intention, or plan. After being advised of the identities of the agents and the purpose of the interview, FERNANDEZ provided the following information: On Friday (August 09, 2019), FERNANDEZ was housed in Cell 218 on L-TIER of the Special Housing Unit within MCC. He received a visit from his girlfriend, TYRELYSHANTI CRIAG, that day. He was suspected of receiving contraband at the visit and was placed in the dry room for approximately 25 hours. FERNANDEZ was then moved to K tier Cell 111. FERNANDEZ last day being housed in L Tier was Friday (August 9, 2019) prior to his visit.
