He is going to 429 East 75th Street, between York and 1'1 Avenue, closer to York. He will register on the 1st floor @ 3:45pm before going to the 2nd floor for his consult and procedure with Dr. Daniel Richman. I am also attaching a 3 page list with medications and instructions, please have Mr. Epstein look it over carefully and follow all that applies to him.
