Hi Karyna,
The form I gave to Jeffrey for you to sign is missing the "6B" part of your Local Physical Address. You
mentioned when you called me back yesterday afternoon that the exact address on your voter
registration card is "6B Little St. James, St. Thomas 00802". The form states that your physical
address should be "as listed on registration card." I have attached a corrected form for you to sign
and FedEx back to me. I need to file this with the Board of Elections by 5PM on Tuesday, October
16th.
