Please find attached all the signed paperwork, driver's license and copy of my last JP Morgan statement with my
Apple Stock. I just have a question for you as to what to put under page 2 of the Account Application in regards
to Initial Transaction Amount, Net Worth, Annual Income, etc.. We can talk whenever is good for you. I'm
assuming this paperwork will allow you to take control of my Apple Stock. How do we proceed with the Note
investment you spoke to us about in Bogota? (Please assume I know absolutely nothing about how these things
work!)
