The denied ETA Form 9035E is attached to this electronic notification and the reason(s) for the denial is listed below. In accordance with 20 C.F.R. 655.740(a)(3), may submit a corrected ETA Form 9035E. In the event that a corrected ETA Form 9035E is submitted, the Department will consider it as a new application and process it on a "first-in, first-out" basis. REASON FOR DENIAL: Section C.12 of this application contains an obvious inaccuracy. The Federal Employer Identification Number (FEIN) value entered in Section C.12 of the ETA Form 9035E could not be verified by the Chicago National Processing Center (CNPC) as a valid nine-digit FEIN assigned by the Internal Revenue Service (IRS). In order for the employer to overcome the issue identified on the denial determination for any future LCAs submitted using this exact FEIN, the CNPC will need to obtain the information below. If you have previously submitted LCAs with this FEIN that have been certified, it is still necessary for you to submit the needed documentation outlined below.
