Attached please find Sainfrans medical history and negotiated payment due. Unpaid amount will be send to
collection agency and after payment plan can be setup with a minimum monthly payment. Darren mentioned
that he will prepare a resolution form to be signed before we make final payment. Please note, that LSJE has
workers Comp insurance that Sainfrans did not claim. There is only 8 days to claim after incident.
