I've filled in the information about the sculpture and Diana Howard has separately sent photos of the work to the Committee, so you just need to fill in the owner info on the 2nd page and check the appropriate box and sign in the space indicated on the 4th page. You will see also on the 4th page that they want a photo ID for you (or whoever ends up signing on behalf of the trust). To be safe, you should also include documentation showing why you are authorized to sign on behalf of the trust. Also, the Committee needs the certificate of insurance, so if it is ready it would be great to send it along with this paperwork. Once you have completed and signed the paperwork and have the attachments ready, you can email it all to me and I'll forward it to the Giacometti Committee. EFTA01060040 The sculpture is being packed today (the outer crate was built over the weekend) and will be on a 10 am flight tomorrow morning December 6th to Liege airport in Belgium. It will arrive at 11:15 pm tomorrow night in Liege and then will travel by truck to Paris. I'm told that it is about 5 hours from Liege to Paris, so the work should be in Paris at the Giacometti Foundation's offices on Wednesday, December 7th. The Committee's meeting is on Friday, December 9th. Please let me know if you have any questions.
