Health Services Please be advised that we require all new students to complete and return health forms. You must also complete certain medical requirements (immunizations, physicals, etc.) Based on certain deadlines and specific time frames for vaccinations, you must start addressing these requirements now. Please go to the following address immediately: www.cumc.columbia.edu/student/health/forrns.html#prereg Go to 2012-13 Pre-Registration Booklet and Forms. Download the forms and complete them in a timely manner. Please remember the deadline is July 13, 2012. You must complete ALL health requirements before arriving on campus (not just submitting forms but also physicals and immunizations). YOU WILL NOT BE ALLOWED TO PARTICIPATE IN CLASSES IF YOU HAVE NOT MET THESE REQUIREMENTS. All students are encouraged to visit the Columbia University Health & Related Services web site to educate themselves about what services are available to students at Columbia University. The address is www.cumc.columbia.edu/student/health. 2. Change of Address/Out of the Country Anyone who has a change of address should notify our office in writing, including the dates the address will be valid, and the new telephone number. Anyone leaving North America should still respond to all email from Columbia University. 3. Visa Status EFTA01176806 Based on your current status: awaiting Asylum decision with 1-589 pending, you can attend CDM without applying for an Fl visa. You should, however, send us copies of your current immigrations status documents (e.g. I-94, passport, copy of I-589, or anything you have available) for our records. 4. Computer Requirements CDM requires that all students own a laptop computer. The computer requirements and other pertinent information can be found at: http://dental.columbia.edu/computers/.
