I 7 TIVE TO 152] I County of Los Angeles, Department of Coroner Investigator?s Narrative Case Number: 2014-05339 Decedent: ABREGO, OMAR Information Sources: 1) Centinela HoSpital Medical Center 310.673.4660 investigation: At 0532 hours on 8/3/2014, Jose Buan, RN called in and reported this apparent undetermined death. was assigned this hospital case on 8/8/2014 by Lieutenant Smith. reviewed the medical records. This death will remain undetermined as the decedent was under the influence of cocaine and he had been in a physical altercation with officers from the Los Angeles Police Department. Location: The injury location is unknown. This death occurred at the Centinela Hospital which is located at 555 Hardy Street Inglewood, CA 90301. Informant/Witness Statements: Medical records contained the following information: On 8/2/2014 the decedent presented to the emergency room with an altered mental status after being taken into custody by LAPD Officers. He forcefully resisted arrest and there was a physical confrontation with the arresting officers. Prior to the arrest it was apparent the decedent's mental status was altered, because when he was pulled over by officers, he jumped out of the back of the vehicle he was driving, and sprinted down street. He appeared to be extremely agitated. Upon arrival to the emergency room the decedent did have evidence of trauma with significant cranial facial swelling and a large laceration over his left eyebrow and multiple confusions and abrasions on his trunk and extremities. After an evaluation the decedent was determined to be critically ill and therefore was admitted to the with the following acute concerns: rhabdomyolysis, acute renal failure, cocaine toxicity, severe concussion with altered mental status, persistent hypoglycemia, facial laceration, multiple facial contusions, and multiple contusions to his trunk and extremities. The decedent remained hospitalized until his death on 8/3/2014 at 0520 hours by Dr. A. Sin. Scene Description: Coroner personnel did not visit the scene, as this is a hospital case. Evidence: . A hair kit was collected by Forensic Attendant Greg Myers at 0045 hours on 8/6/2014. Body Examination: The decedent appears to be an adult Hispanic male with brown hair, brown eyes, natural teeth, and an unshaven beard and mustache. He was nude and lying supine on a steel table in the F80. The only medical appliance still in place was a catheter. Sutures were noted above his left eye. Bruising was present on his left eye. Abrasions were noted on the right side of his face, the right side of his chin, above his right eye, and on his right knee. Tattoos observed was a woman's face with on his left arm, a flower with a woman's face and on the left side of his chest, and a tribal band on his right bicep. Two vertical scars were noted on his right knee. He was transferred to Centinela Hospital Medical Center where he was diagnosed with rhabdomyolysis, acute renal failure and cocaine toxicity. Despite medical intervention he was pronounced dead on 8/3/2014 at 0520 hours. BLUNT FORCE INJURIES: Blunt force injuries of face: Examination of the face reveals multiple injuries. There is a shaped laceration above the left eyebrow with the long arm measuring lwl/Z inches and the short arm measuring inches. is subcutaneous hemorrhage associated with this injury. fracture or brain hemorrhage is noted. is a inch red abrasion. EXTERNAL EXAMINATION: The body is identified by toe tags and is that of an unembalmed, refrigerated adult male Hispanic who appears about the reported age of 37 years. The body weighs 180 pounds, measures 64 inches and appears moderately obese. Well?healed scars are noted to the anterior and medial right knee. Multiple tattoos are noted to the upper back, left upper chest and left upper arm- Also noted are purple ink markings on the left lower back and posterior left upper leg. These markings appear hospital related. Rigor has presumably been abolished. The head is normocephalic and covered by black hair. There is no balding and the hair can be described as short and wavy. Mustache and beard are absent. Examination of the eyes reveals irides that appear to be brown in color and sclerae that are congested. There are no petechial hemorrhages of the conjunctivae of the lids or the sclerae. The oronasal passages are unobstructed. Upper and lower teeth are present. The neck is unremarkable. There is no chest deformity. There is no increased anteriorwposterior diameter. The abdomen is obese. The genitalia are those of an adult male. The penis appears circumcised. The external genitalia are without trauma or lesions. The extremities show no edema, joint deformity, abnormal mobility or needle tracks. EVIDENCE OF THERAPEUTIC INTERVENTION: The following are present and are in proper position: tube, endotracheal tube, fossa and left wrist, Nasogastric intravenous lines in the left antecubital venipuncture marks in the right antecubital EMF COUNTY OF LOS ANGELES DEPARTMENT OF CORONER AUTOPSY 2014*05339 FIGUEROA, OMAR 1%:ch fossa and right wrist. There is evidence of old surgery. Scars are present to the right knee. Orthopedic hardware is present. There has not been postmortem intervention for organ procurement. CLOTHING: The body was not clothed and I did not see the clothing. INITIAL INCISION: The body cavities are entered through the standard coronal incision and the standard Ymshaped incision. No foreign material is present in the mouth, upper airway and trachea. EVIDENCE OF INTERNAL INJURIES: There is a focal 1 inch area of hemorrhage to the mid right supraclavicular soft tissue. NECK: The neck organs are removed en bloc with the tongue. No lesions are present nor is trauma of the gingiva or oral mucosa noted. There is no edema of the larynx. Both hyoid bone and larynx are intact and without fractures. No hemorrhage is present in the adjacent throat organs, investing fascia, strap muscles, thyroid or visceral fascia. There are no prevertebral fascial hemorrhages. The tongue when sectioned shows no trauma. CAVITY: Both pleural cavities contain no fluid or adhesions. No tension pneumothorax is demonstrated. The parietal pleurae are intact. The lungs are voluminous. Soft tissues of the thoracic and abdominal walls are well preserved. The organs of the abdominal cavity have a normal arrangement and none are absent. There is no iluid collection. The peritoneal cavity is without evidence of peritonitis. There are no adhesions. SYSTEMIC AND ORGAN REVIEW The following observations are limited to findings other than injuries, if described above. MUSCULOSKELETAL SYSTEM: No abnormalities of the bony framework or muscles are present. CARDIOVASCULAR SYSTEM: The aorta is elastic and of even caliber throughout with vessels distributed normally from it. Both abdominal and thoracic aorta have no atherosclerosis. There is no tortuosity or widening of the thoracic segment. The abdominal aorta has no atherosclerosis. There is no dilation of the lower abdominal segment. No aneurysm is present. The major branches of the aorta show no abnormality. Within the pericardial sac there is a minimal amount of serous fluid. The heart weighs 360 grams. It has right ventricular hypertrophy. The right ventricle is 0.6 cm thick and the left ventricle is l.2 cm thick. The chambers are normally developed and are without mural thromboses. The valves are thin, leafy and competent. The circumference of the valve rings are: Tricuspid valve ll cm, pulmonic valve 7.2 cm, aortic valve 6.8 cm} mitral valve 11.2 cm. There is no endocardial discoloration. There are no lesions of the myocardium. There is no abnormality of the apices of the papillary musculature. There are no defects of the septum. The great vessels enter and leave in'a normal fashion. The ductus arteriosus is obliterated. The coronary ostia are widely patent. There is no coronary atherosclerosis of the major coronary arteries. No focal endocardial, valvular or myocardial lesions are seen. The blood within the heart and large blood vessels is liquid. SYSTEM: A moderate amount of edema is found in the lower bronchial passages. The mucosa is injected throughout. The lungs are subcrepitant and there is dependent congestion. The left lung weighs 600 grams. The right lung weighs 800 grams. The visceral pleurae are smooth and intact. The is congested and edematous. The pulmonary vasculature is without thromboembolism. GASTROINTESTINAL SYSTEM: The esophagus is intact throughout. The stomach is not distended by gas. It contains 5 cc of chalky green fluid. The mucosa is unremarkable and without trauma. No tablets or capsules are identified. The small intestine and colon are opened along the anti?mesenteric border and are unremarkable and without trauma. The appendix is present. The pancreas occupies a normal position. There is no trauma. HEPATOBILIARY SYSTEM: The liver weighs 1990 grams. It is enlarged and is red? brown. The capsule is intact and the consistency of the is firm. The cut surface is smooth. There is a normal lobular arrangement. The gallbladder is present. The wall is thin and pliable. It contains approximately 5 cc of bile and no calculi. URINARY SYSTEM: The left kidney weighs 140 grams. The right kidney weighs 160 grams. The urinary bladder is unremarkable. It contains no urine. GENITAL SYSTEM (MALE): The prostate is without enlargement or nodularity. Both testes are in the scrotum, are unremarkable and without trauma. SYSTEM: The spleen weighs 125 grams and is of average size. The capsule is intact. The is dark red and mushy. There is no increased follicular pattern. nodes throughout the body are small and inconspicuous. The bone is not remarkable. The bone marrow of the rib is red and moist. ENDOCRINE SYSTEM: The thyroid is unremarkable. The parathyroid glands are not identified. The adrenals are intact without necrosis or hemorrhage. The thymus is not identified. The pituitary gland is unremarkable. SPECIAL SENSES: The eyes, middle and inner ears are not dissected. HEAD AND CENTRAL NERVOUS SYSTEM: There is subcutaneous and subgaleal hemorrhage deep to the laceration of the left forehead. The hemorrhage extends into the left orbit but not the temporal muscles. There are no tears of the dura mater. There is no epidural, subdural or subarachnoid hemorrhage. The brain weighs 1275 grams. The leptomeninges are thin and transparent. normal convolutionary pattern is observed. Coronal sectioning demonstrates a uniformity of cortical gray thickness. The cerebral hemispheres are symmetrical. There i no softening, discoloration or hemorrhage of the white matter. The basal ganglia are intact. Anatomic landmarks are preserved. Cerebral contusions are not present. The ventricular system is unremarkable without dilation or distortion. Pons, medulla and cerebellum are unremarkable. There is no evidence of uncal or cerebellar herniation. Vessels at the base of the brain have a normal pattern of distribution. There are no The cranial nerves are intact, symmetrical and normal in size, location and course. The cerebral arteries are without arteriosclerosis. A SPINAL CORD: The entire cord is not dissected. HISTOLOGIC SECTIONS: Representative sections from various organs are preserved in one storage jar in 10% formalin. TOXICOLOGY: Bile, blood, liver tissue, stomach contents and vitreous humor have been submitted to the lab. A comprehensive screen was requested. RADIOLOGY: The body is fluoroscoped and x?rays are taken of the head, chest and extremities. No fractures are identified. WITNESSES: Detective Grabiak and Detective Forsman of the LAPD Department Force Investigation Division and Mr. Mike Moursalian of the DA's office witnessed the autopsy. DIAGRAMS USED: Diagram Form 20 was used during the performance of the autopsy. The diagram is not intended to be a facsimile. OPINION: The decedent?s bizarre behavior and agitation is a result of cocaine intoxication. During the attempt to place the decedent into custody on 08/02/2014 the decedent sustained a laceration of the forehead by striking head on the ground (self?inflicted by hiStory). He did not lose consciousness at the scene of arrest. He was transferred to Centinela Hospital Medical Center where he was diagnosed with rhabdomyolysis and acute renal failure. He was pronounced dead at the hospital 08/03/2014. Autopsy revealed injuries consistent with blunt head trauma and nonspecific abrasions of the chin, arms and legs. Internal examination revealed moderate atherosclerotic heart disease. Toxicology results performed on blood samples taken at autopsy show the presence of cocaine and its metabolite benzoylecognine.
