OCME case #M19019432 Were all on scene at the Manhattan Correctional Center at 1200 hours and allowed into the facility. Epstein was discovered unresponsive alone in his cell #220 9th floor L tier at approximately 0633 hours. BOP Michael Thomas. According to information obtained from BOP Special Investigator Lieutenant T. and information that was related to her, Epstein was found with a torn piece of issued BOP orange colored bedding around his neck with the other end affixed to the bunk bed. CPR was conducted and Supv . BOP nurse/medic was alerted and responded. who was interviewed by C19 personnel stated that Epstein was unresponsive and had no vitals at 0635. attempted to shock Epstein with and AED but no shock was advised. Epstein was transported to the BOP medical room on the second floor with CPR still in progress and oxygen along with other narcotics administered with no response. 911 was called and records indicate Epstein was transported at 0644 hours to NY Presbyterian where he was later dead. Office of the Inspector General has been made aware that a BOP guard falsified logs regarding to Epstein. Details of who or how this information was exactly obtained is still being gathered but preliminary information is that an officer made a confession to some authority. OIG has opened up a criminal case in regards and has EFTA01659612 asked all interviews or scheduling of interviews be suspended and coordinated with their offices. Upon receipt of the BOP complaint to OIG, the BOP complaint stated that 2 guards falsified the logs indicating that they conducted their mandatory rounds (which are to be conducted every 30 minutes) when in fact they did not. Upon further receipt and review of the complaint provided by to OIG C19 personnel will coordinate and investigate with CHG. Background: *07/06/2019 Epstein arrived at BOP MCC *07/08/2019 Epstein placed on suicide watch and spoke with psychologist and underwent observations until 07/10/2019 *07/23/2019 Epstein 1st suicide attempt via hanging at 0127 hours, cellmate at the time. Placed on special observation until 07/29/2019. *08/09/2019 Epstein cellmate was brought to court a d released from court, leaving Epstein alone in his cell *08/09/2019 at 1919 hours BOP allows Epstein to make a phone call on an unsecured and un recorded line to Karyna Shuliak. That call lasted 19 seconds and details of that call are unknown. SDNY is working with Shuliak lawyer to determine meeting. *BOP records show no visitors to Epstein since entering *BOP records show no calls by Epstein since entering. Epstein also listed no numbers or persons of contact to BOP upon his initial admission *Video surveillance of the hallway that captures the cell Epstein was housed in was reported "broken" on Thursday *8/8/2019 with no record made.
