I will send you Jeffrey's 10 year passport on Monday for Tuesday delivery. I need to keep one
passport with him at all times-so need to wait until the 4 year is back in house! (and you arc
closed Monday anyway!)
I also have the 2 Letters of Authorization printed out with only the 2 boxes checked off with his
signature...will include with the 10 year passport.

with the applicant's original (pen-on-paper) signature & date in blue or black ink

Original Primary (10 year) Passport
We can ship back the Second Valid Passport at your request, but will hold on to it until
you advise us to do so.
