I am attaching our PMA contract which I need signed and returned BEFORE your consultation. This allows the doctor to freely speak with you about all the alternative therapies that we offer. The $10 for your PMA contract will be added to your visit. Also, please send over PRIOR to your consultation the following: 1. A DETAILED summary of your health from date of diagnosis to current. This is the most useful information for the doctor as they want to see where you are at now. 2. They prefer not to review hundreds of pages of records from the past. If you have a report from a current scan and current blood work please email or fax ahead of time. We do not accept CDS 3. If you haven't already provided your list of your supplements and medications we will need that as well It is essential that we have these items sent over 3 days prior to your consultation for the doctor's review.
