To: Cc: Ir j From: Sent !2015 4:27:13 PM Subject and Medical Form Hi Courtney...I dropped off the medical form for football to Dr. Pierce but was told he is extremely backed up...I asked the receptionist to please have the form filled out and signed by this Friday so I can pick up football equipment and was told they will do their best, but it is possible I will not have by Friday... Please, may we still pick up equipment this Friday? I will have the $200 check with me for sure...if I on't have the medical form by Friday I am positive I will have early next week and can scan and email to you same day...
