It is located in the same envelope it arrived in. I wrote on the envelope "MRI, Dec. 15, 2016, Dr. Moskowitz" The envelope is in the file cabinet under my desk in a hanging folder more toward the back of the cabinet....it is the fullest folder in there-and it should be the envelope in the front of everything else since it is the latest item to go in. Can you please get the whole envelope and send to JE via Fed Ex to PB. Please provide me the tracking number once done. It needs to go to him today, overnight, fastest way.
