I propose a yearly retainer contract for the sum of $25,000 for two you. This includes all
of your medical services and lab work performed in the office aside from infusions,
house calls, and injections. In addition you will have full email and phone access and all
services will be submitted to your insurance, paid to you. This yearly retainer will be
