The web portal for =pplying is www.ciciutility.org . On this =eb site is a database of all entities who have registered for the "CICI =dentifier". The database shows the name, address, entity form and CICI =dentifier for each registered entity. JP Morgan is requesting =hat we provide a CICI identifier for FTC by December =,2012. The registration fee is $200 per entity. A $100 yearly =aintenance fee will also be =harged.
