Th count slips (for R&D and the SHU) should have been trashed, and new counts slips should have been generated. Page 80: Never knew the counts slips were off and this was a problem until now. Page 83: Just didn't remember what took place and what happened. Page 84: Said the 4 pm, 10 pm, and 12 am count slips provided by the SHU were all bad/inaccurate because was moved to the R&D holding cell at 3:15 pm but was never removed from their count. However, in rare instances, there have been inmates who were WAB and didn't leave. Page 100: : But if by, for instance, he leaves at 8:00, he's WAB, by 2:00 p.m., if he's not back, that means, certainly, he got off on the transport. Does that mean, basically, by 2:00 p.m., if somebody that's WAB is not back, they're certainly not returning? MR. =I: Yes. Okay. And in this case, if someone is WAB, would control have any — or internal - have anything to do with, at the 4:00 p.m. count, contacting SHU, to make sure that they're aware that that WAB person, in fact, isn't coming back? MR. =: No. Page 115: Control may not have been notified about so they may not have known to key him out of the SHU. It should have happened at 3:15. The SHU should have been notified and then should have contacted control and told them to make the necessary key entries. Page 119: If the SHU conducted the 4 pm count, they would have caught the mistake. Page 125: Do not know for a fact that the handwriting on the SHU counts slips occurred during the 10 pm count. But pretty positive it was written during the 10 pm count. Page 126: : Well, it sounds like you knew it. I'm just - now, I'm just super confused if that's when it happened, why the El, you have a count slip that doesn't say on the El. Upon my arrival I was informed that an inmate had attempted suicide and proceeded to cell, Z05-124LAD. I observed inmate Epstein, Jeffrey #76318-054 lying in the fetal position on the floor of his cell wearing a t-shirt and boxers. He was breathing heavily and was snoring. I called out to inmate Epstein and observed him flicker his eyes and continue snoring. His neck was red with no abrasions. I observed no further injuries to his person. An attempt was made to get the inmate to stand on his own with negative results. The inmate was placed in hand restraints and staff was directed to retrieve the stretcher. As inmate Epstein was be
