Since his intake on 7/8/19, he received four suicide risk asses sments. One resulted in a one day suicide watch following the likely attempt we discussed. After the one day on watch he spent a week on psychological observation. He was taken off psyc observation on the 30th and seen ag ain on the 31st. On the first he returned from court and the USMS form noted suicidal ideation, so he was assess ed for suicide again, but not placed on watch. He was last seen on the 8th, and no significant concerns were note d at that time. I anticipate we will have some recommendations from the reconstruction, but also see that the tea m in NYC was working to accurately assess and protect him while balancing that with his need for environment t hat promoted some quality of life.
