Targeted victim was approached by a wo
' a clothing store located in the Kadina district
while shopping with her mother, Cynthia
and asked if she would be interested in doing
modelling as a Kmart catalogue model in ma catalogues.
The targeted victim replied "I will have to ask my mother" , and proceeded to walk towards her
mother while the unknown woman waited for her to return.
The targeted victim approached her mother and informed her mother of what the woman had
proposed regarding becoming a model for Kmart catalogues.
The targeted victims mother informed her daughter,
(targeted victim) that she
could not allow her daughter to do modelling due to nancia reasons and that she was raising a
family of four children and could not support her daughter by accompanying her to modelling
shoots, which would be necessary as she was a minor of 14 years of age.
The targeted victim returned to the woman who stood waiting for her and informed her that she
would have to decline the offer of doing modelling.
The woman smiled at the targeted victim before leaving the clothing store.
The woman was unknown to the targeted victim and was not sighted in the Kadina district again
following this incident by the targeted victim.

I was contacted
2021 by Officer Leong, stating that she was from Services SA on the
phone number ISDuring
this phone contact I was informed that the phone call was being
Recorded and the information given was not to be shared with a thin' party during the recorded phone call.
Officer Leong (woman) informed me of the issue of suspected Identity Theft (my Identity) which involved
numerous bank accounts being opened in my Identity and being used for Illegal purposes of Drug
Traffieldng, Money Laundering, Money Transfers to a corporation, Fraudulent Activity and were operating
in numerous locations, from recalled memory being Queensland, ANZ, NAB, as some of the numerous
accounts, and involied over I million dollars illegally rotating through the bank accounts.
I was inflamed that my Tax file number was being Illegally misused in Australia and that my bank accounts
and tax file number would be suspended.
I was asked to provide the details of my banking institutions and the balances ofthe bank accounts in my
name. I informed Officer Leong that I held 3 bank accounts with the Commonwealth Bank, 2 personal
accounts and 1 business accounts, balances being $530.00, Goal Saver account, $10.00 - $15.00 in the
business account and $15.00 - S20.00 in the personal account I informed Officer Leong that the accounts
were set up by the Commonwealth Bank Manager, Paul Flavel, Katrina and could be confirmed by Paul
Flavel, Manager.
Following providing my bank account details I informed Officer Leong that this was a case of Identity

Officer Leong then informed me that my bank accounts and Tax file number would not be subjected to a
suspension and the case would be referred to the Federal agencies for athorough investigation and I was
not instigated in the illegal activity that had occurred because ofthe Identity Theft -Legal Investigation.

over a. 10 year period, the source of the continuous Fraudulent Activity in every account server has been a
result of Remote Access to servers, accounts and Computer Hardware resulting in blocked access, blocked
contact altered details and Fraudulent Activity and has been conducted by Government and Police Sources

I reported The Fraudulent malicious Activity of the newly created business account to Joshua
Adams, Business Consultant, Telsira Store Nadine who proceeded to file a report to the Fraud
Department and requested an investigation into the activity - Refer to Telstra Report dated
1/10/2020
I received no reply or response from this report
The Fraudulent and malicious Activity continued and escalated.
On the 25/03/2021 I reported the activity again to Joshua Adams, Business Consultant Telstra
Store, Kadina and reported that I had not received any contact from the Fraud Department
concerning the report that he had submitted. I requested a change of mobile phone number with
the account still having Restricted Access applied. Joshua Adams proceeded to change the mobile
phone number and did a. check to ensure that the new number that I had been given was a newly
created number that had not been circulated before my use, to ensure that the same activity would
not occur end if it did would be seen as suspicious activity continued.
On the 25/0/2021 I reported the Fraudulent Malicious activity to Isabelle Oppeloar, Manager,
Telstra Store, Kadina who proceeded to file a report to a higher authority, requesting an
investigation with the higher authority of the. Fraud Division or CEO of Telstra, due to ongoing
issues over

On the 13th Apri12O21 I sent a letter addressed to the CEO of Telstra Head Office, Mr Andrew
Penn, Melbourne, yictoria from the Norwood Post Office branch, Adelaide informing the CEO of
the continuous Fraudulent Malicious Activity that had been occurring in my personal and newly
created business account with Restricted Access, and requested a thorough investigation into the
activity and provided the documented evidence of the activity that had been occurring in all
servers and over a long period of time, and following re-location and change of details

Brief Overview
At about 11.32 pm on Tuesday, 19th May, 2015, at Port Pixie the accused,
drove a motor vehicle namely a Mitsubishi sedan
on a road namely The Terrace, whilst she exceeded the prescribed concentration

of alcohol, reading being 0.050.
Police
At about 11.32 pm on Tuesday, 19th May, 2015, Police (O'Neill/While) were on
mobile uniform patrol when they stopped the accused who was driving a Mitsubishi
sedan
south along The Terrace, Port Pixie for a random breath test.
At 11.33 pm the same date the accused submitted to an alcotest and gave a
positive reading.
The accused was conveyed to the Port Pixie Police Station, where at 12.02 am on
Wednesday, 20th May, 2015, the accused submitted to a breath analysis
instrument. These two blows were not accepted by the breath analysis
instrument.
At 12.15 am the same date, the accused again eub
breath analysis instrument and gave the reading

JACOBS states that at about 8:30 pm on Thursday 13th August 2015 she was about
50 metres away from the Jetty at a place known to the PORT PIRIE locals as "The
Bridge to nowhere". JACOBS was with friends doingsi

and the group she was with.
JACOBS watched as the sedan drove towards them at speed. JACOBS didn't know what
speed it was travelling at but thought it was going so fast it was going to hit
her and the group.
JACOBS states that the car braked heavily and turned, stopping about ten (10)
metres away from her. The car was being driven in a very erratic manner.
JACOBS observed that the driver of the sedan wan a female, wearing a blue dress
with high heels and blonde hair and that she was the only occupant Of the
vehicle. JACOBS Saw that she exited from the driver's seat.
JACOBS further states that she could hoar the female talking but could not see
anyone else with her, she wasn't sure if the female was talking to herself or
not.

At about 9:07 pm on Thursday 17th August 1015, Police KELLY was on mobile
uniform patrol when tasked to a Driver Under the Influence in an area of PORT
Plan known as °The Bridge to Nowhere* on the John Pirie Island. The tau

and that the driver bad almost been involved in an accident.
On arrival Police KELLY in a marked police vehicle located the motor vehicle in

KELLY observed a female seated in the driver's seat of the sedan and exit the
vehicle to be standing next to the driver's door on KELLY emitting his valicla.

I turned the vehicle off which turned off the music in the car. I secured the keys that were
located in the ignition to my key clip on my work belt. The key was a single MITSUBISHI
car key with a black tag attached.

At the time of her arrest,
iwas holding in her hand a can of "Wild Turkey"
bourbon and a cigarettea
smelt strongly of intoxicating liquor during my dealings
with her.

A bag was located inside the vehicle which was conveyed with
to the Port Pirie
Police Station.

At the Port Pirie Pollee Station,

I am providing this statement to Constable Scott KELLY of the Yorke Mid Highway Patrol
per phone.

I provided this addendum statement on Thursday 11th of August 2016.

While I was doing my photography with a group of friends over °The Bridge to Nowhere"
in Port Pirie, I observed a vehicle come up to us at speed then skid to a stop. I was
worried that the car was going to crash into us. I went up and spoke with the driver. It
was a female and she was alone in the vehicle.
4. .t)frShe exited the vehicle and it appeared as though she was having a one person party.
She was lounging on the bonnet of the vehicle and dancing around.
5.
One of the group contacted police and when they came, they arrested her. It was the
same woman that was driving that was arrested by police.

prevents you from providing a breath sample, but only if you ask for a sample of your blood to be taken instead,
or can show that your condition also prevents the taking of blood. If you want a blood sample taken because
of your condition, you should asE for that and the police will help you to have the sample taken at Government
