Parliament Rob Lucas sworn in as an interim three-person government by the Governor of South

Officer Leong (woman) informed me ofthe issue of suspected Identity Theft (my Identity) which involved

TrefEektialoney Laundering, Money Transfers to a corporation, Fraudulent Activity and were operating
in numerous locations, fromiecalled memory beth.ggneensland, ANZ, NAB; as some of the numerous
accounts, and involved over 1 million doBaisfilsallir rotating/throughthe bank =our&
I was informed that my Tax file number was being Illegally misused in Australia and that my bank accounts
and tax file number would be suspended_
I was asked to provide the details of my banking institutions and the balances ofthe bank accounts in my
name. I informed Officer Leong that f held 3 bank scampi's wall the Commonwealth Bank, 2 personal
accounts and I business accounts, balances being $530.00, Goal Saver account, $10.00 -$15.00 in the
business account and $15.00 - $20.00 in the personal 8CCOIDit unformed Officer Leong that the accatmts
were set np by the Commonwealth Bank Manager Paul Flavel, Kadin& and could be confirmed by Paul
Flavel, Manager.
Following providing my bank account details I informed OfficerLeong that-this was a case of Identity

She has suffezed severe reactions to several medications previously, including Olanzapine, Escitalopram,
penicillins and Champix.
Furthermore claims to have suffered anaphylaxis to a flu vaccine in 2014, and respiratory symptoms following
an ADT vaccine in August 2020.
She is understandably concerned about having a serious reaction to the COVID-19 vaccines. As a consequence
I referred her to the RAH for administration of the COVID-19 vaccine. She was administered the Pfizer
vaccine, and 5-7 days alter developed facial swelling and urticarial rash and difficulty breathing. She was
treated with a combination of prednisolone and anti-histamines.
I would appreciate your review and opinion to better understand the nature of these events and understand
whether there is an allergy to possibly a preservative in the vaccine, and if so, her suitability for further
COVID-19 vaccines.

Dear Dr Richard Harries,
I am contacting you in hope that you are the cosmetic surgeon that I have been looking for for 20 years. I am not sure
if you remember me but I was one of your firstpatients of-
ur career of which you conducted cosmetic surgery at the
Wallaroo Hospital in the year 2001. My name is
although at the time of the surgery I was married
and was under my married name of McDiarmid.
At that time when I had the first
consultation with you we discussed that you would be willing to do the surgery on the basis that I would allow you to
take photographs of the surgery following the procedure as a basis for the beginning of your career to represent your
work and expertise to your patients. Upon this basis I agreed and the procedure was preformed. I then attended two
appointments following the procedure which you did take the photographs according to our agreement. Unfortunately
due to the abusive man that I was married to at the time,who was also very controlling and ordered that I was not to
attend the third and last appointment or I would be physically abused I did not attend the final appointment, and I was
not able to complete our agreement.
I have been carrying this guilt of letting you down for the past 20 years
and did not know how to find you following this event, as I'll be honest all paper work was destroyed containing your
name by my husband, who was suspicious of a man, a doctor making this agreement with his wife and the reason I
could not attend the final appointment, his jealousy of a man being nice to me, which this kind of treatment continued
for the 17 years I was married to him resulting eventually in my isolation from society, until I divorced him in 2007, and
got a restraining order against him.
I have recently been given your name on a medical record obtained from Dr Richard Daehn, Kadina Medical
Association and could not believe my luck. I finally found you.
I am contacting you to inform you of the circumstances of that event, and the fact that I always felt I had let you down
and wanted to ask you if we could complete the agreement and take the last photograph of your brilliant work even
though it is 20 years later, but as you stated then I would need to maintain the surgery repair and I have done just that
with exercise over the last 20 years and the work that you performed is still brilliant and would be of great be I think to
show your patients of the lasting effects of your brilliance.
Could you please contact me to inform me if you would like to proceed with this option of completing the photographs
of your work and if you remember me and would like to , I will then proceed to make an appointment for the next
available appointment at the Wallaroo Hospital if you are interested where it all began, as I have now relocated back
to Kadina.
I would appreciate it if you would allow me to redeem myself by allowing me to complete the agreement as I felt
terrible all these years for letting you down.

I refer to your complaint to the Oftice of the Health and Community Services Complaints Commissioner (HCSCC) about your experience at Wallaroo Hospital.
On 11 November 2022, the HCSCC commenced pie-conciliation inquiries into your complaint under section 30 of the Health and Community Services
Complaints Act 2004 (SA) (the Act).
Yorke and Northern Local Health Network (YNLHNys response

• You attended the hospital three limes on 18, 22 and 23 February 2022.
• On 18 February 2022, you attended the hospital at 09:05hs and a registered nurse reviewed you at 09:20am. All observations were normal. You were 10
days after a COVID-19 vaccine administration and the doctor prescribed cetirizine and antihistamine as a treatment for allergic reaction. The allergic
reaction was thought to be due to a viral infection and the medication prescribed followed the clinical pathway.
• On 22 February 2022, you presented at 17:25hs and were seen at 18:34hs. Your observations were again within normal parameters, except for a slightly
elevated pulse rate. M ECG was taken and shunt i to the doctor. The doctor was notified of your presence in the department at 19:00hs. At that time,
the doctor had multiple patients waiting and it is up to the doctor to pnoritise appropriately based on clinical need of all patients. You decided to leave at
19:45hs because you were feeling better. This was against clinical advice to stay. You were advised to retum if concerned.
• On 23 February 2022, you anived to the ED at 8:31hs and were seen by a nurse at 8:4541s and by a doctor at 8:52hs. Again, the observations were
within normal parameters apart for a slightly elevated heart rate. Examinations showed a swollen face and rash with anxiety. The diagnosis of allergy
was mace again and a short, reducing course of steroids was proscribed following clinical pathway.
• There are four types of allergic reaction. The type IV reaction is a delayed reaction which takes up to 72 hours to develop. A longer time to develop
reactions can occur but Is a different type of reaction, such as a granulomatous reaction. This is why the first doctor made a diagnosis of an allergy due
to viral infection and not the COVID-19 vaccination which was administered 10 days prior to the first attendance.
• There are no records of Incidents or staff's poor behaviour or attitude during your presentations.
• Unfortunately, the hospital's Director for Nursing and Midwifery did not reply to your first letter.
• After an Investigation Into your complaint. YNLHN is confident that your treatment was appropriate.
• YNLHN acknowledges that the waiting time of your second visit was longer than the hospital would like. However, your observations were normal and
you would have been seen by a doctor if you had chosen to stay.
• YNLHN also acknowledges that you felt your treatment could have been better.
On 13 February 2023, the HCSCC sought a response from YNLHN as to whether they would consider your request for compensation.
On 1 March 2023, YNLHN advised the HCSCC they decline to consider your compensation request because:
• The COVID-19 vaccine was not administered by them; and
• The care provided for the subsequent reaction was appropriate.
YNLHN further advised us consumers who have experienced harm from COVID-19 vaccine can seek compensation via the COVIS-19 vaccine claims scheme:
COVID-19 vaccine clVms scheme - Services Australia.
HCSCC's preliminary views
The HCeir-C is 01 the view YNLHN has provided appropriate information and explanation to address your complaint. The HCSCC's conciliation is a voluntary
process and we are unable to seek compensation if YNLHN declines to participate in the process.
In light of the above, the HCSCC considers your complaint has been addressed to the extent It can be.

I am contacting you to submit the final outcome of the medical complaint I submitted to the complaints commission concerning the Wallaroo Hospital.
As detailed in the forwarded email I received from the complaints commission the Wallaroo Hospital is under the influence and directorship of Mr Roger
Kirchner of the Port Pale district Hospital of which he is also director of. As we all know, and as 8 previous resident of Port Pire the Port Pirie Hospital is
operating at a proficiency not seen in the Wallaroo Hospital but are under the exact same leadership, Influence and input of Mr Roger Kirchner, which does
ding into question why there are so many issues concerning the operations of the Wallaroo Hospital and our community members are preferring to attend the
Port Pirie Hospital.
I am submitting the received outcome of my complaint with the complaints commission containing Mr Roger Kirchners denied complaint involving the Wallaroo
Hospital and the near fatal incident that I suffered due to the Inefficiency of the Wallaroo Hospital emergency department that is under the directorship and
influence of Mr Roger Kirchner, and submit this report in support of your medical government parliamentary petition to detach the Watford) Hospital from this
influence and directorship and to reform and restructure the Hospital to be under the operations of our communities independent influence and directorship.
I submit this report in a 100% support of your office and the marvellous work you are achieving in getting this issue recognised and restructured to servo the
members of our community in a proficient manner that will no longer suffer from the ongoing complaints and crucial issues regarding healthcare in our
community.

I refer to your complaint to the Office of the Health and Community Services Complaints Commissioner (HCSCC) about your experience at Wallareo Hospital.

• You attended the hospital three times on 18, 22 and 23 February 2022.
• On 18 February 2022, you attended the hospital at 09:05hs and a registered nurse reviewed you at 09:20am. All observations were normal. You were 10
days after a COVID-19 vaccine administration and the doctor prescribed cetirizine and antihistamine as a treatment for allergic reaction. The allergic
reaction was thought to be due to a viral infection and the medication prescribed followed the clinical pathway.
• On 22 February 2022, you presented at 17:25hs and were seen at 18:34hs. Your observations were again within normal parameters, except for a slightly
elevated pulse rate. An ECG was taken and shown to the doctor. The doctor was notified of your presence in the department at 19:00hs. At that lime,
the doctor had multiple patients waiting and it is up to the doctor to prioritise appropriately based on clinical need of all patients. You decided to leave at
19:45hs because you were feeling better. This was against clinical advice to stay. You were advised to return if concerned.
• On 23 February 2022, you amved to the ED at 8:31hs and were seen by a nurse at 8:45hs and by a doctor at 8:52hs. Again, the observations were
within normal parameters apart for a slightly elevated heart rate. Examinations thawed a swollen face and rash with anxiety. The diagnosis of allergy
was made again and a short, reducing course of steroids was prescribed following clinical pathway.
• There are four types of allergic reaction. The type IV reaction is a delayed reaction which takes up to 72 hours to develop. A longer time to develop
reactions can occur but is a different type of reaction, such as a grarnlomatous reaction. This is why the first doctor made a diagnosis of an allergy due
to viral infection and not the COVID-19 vaconation which was administered 10 days prior to the first attendance.
• There are no records of incIdents or staffs poor behaviour or attitude during your presentations.
• Unfortunately, the hospital's Director for Nursing and Mictivifcry did not reply to your first letter.
• After an investigation Into your complaint, YNLHN is confident that your treatment was appropriate.
• YNLHN acknowledges that the waiting lime of your second visit was longer than the hospital would like. However, your observations were normal and
you would have been seen by a doctor if you had chosen to slay.
• YNLHN also acknowledges that you felt your treatment could have been better.
On 13 February 2023, the HCSCC sought a response from YNLHN as to whether they would consider your request for compensation.
On 1 March 2023, YNLHN advised the ticscr they decline to consider your compensation request because:
• The COVID-19 vaccine was not administered by them; and
• The care provided for the subsequent reaction was appropriate.
YNLHN further advised us consumers who have experienced harm from COVID-19 vaccine can seek compensation via the COVIS-19 vaccine claims scheme:

The HCSCC is of the view YNLHN has provided appropriate Information and explanation to address your complaint The HCSCC's conciliation is a voluntary
process and we ere unable to seek compensation if YNLHN declines to participate in the process.
In light of the above, the HCSCC considers your complaint has been addressed to the extent it can be.

How to interpret this chart
This table shows a summary of the status of ail inpatient beds in the Port Augusta
Hospital (PANS).
In an online environment, you can dick on the chosen hospital in the table and the whole
dashboard will update to show data specific to that hospital.
The dashboard displays occupancy per hospital split across 'general' beds and 'all' beds,
breakdown of inpatient, the length of stay, and much more.
Professor Moyes said the
dashboard, which is available
online, will be updated every
30 minutes to provide real-
time information about bed
occupancy, average length of
stay and inpatient flows.
"The dashboard also includes
information specific to each
hospital about the number
of patients currently within
each area or clinical unit at
the hospital," Professor
Moyes said.
"It allows people to easily see
all of the different 'streams'
of inpatient care that each
hospital provides, such as
surgical or pediatrics, and the
status of each of these areas.
"Staff already use the
dashboard, along with a
range of other tools, to help
monitor and manage capacity
and ensure that patient safety
is maintained.
