Polina, Jeffrey had a 2nd passport once before and I see a file in Darren's File Cabinet #2 in the JE misc file (drawer 3). You may find a previous letter that Darren has written for this and Darren could simply review the letter, make changes and you could type up(?) The website looks to be quite helpful.
