Attach is Mr. Epstein statement. As I mentioned over the phone UHC paid the doctor $380.00 but since we are out of
network they have sent that check to the patient. Please forward the check to us as soon as you receive it. The reminding
balance of $95.00 was applied toward Mr. Epstein co-insurance.
