Zip: 00802 Phone: This change order is in reference to the contract dated January 19 2012 signed by Rex Wolterman of Virgin Island Gunite Pools and Brice Gordon of LSJ, LLC.: Work to be performed at: Little St. James Main Pool Email: • Chip•out, bond coat, and re-plaster the main pool - LSJ is to provide all materials to the site of the main pool The above work to be performed in accordance with the drawings and specifications submitted for above work and completed in a substantial workmanlike manner for the sum of: S 28,000.00 — Twenty eight thousand dollars With payments to be made as follows: S 14,000.00 due prior to bond coat S 14,000.00 due upon filling pool with water Any alteration or deviation from above specifications involving extra costs will be executed only upon written orders and will become an extra charge over and above the estimate. All agreements contingent upon strikes, accidents, or delays beyond our control. Homeowner to carry fire, tornado, and all other necessary insurance upon above work.
