us. we may ask for the following information: 1) your name, address and account number:2) the name of the person receiving the funds. and if you know it. his or her telephone
number andfct address: 3) the dollar amount of the transfer. a) the reference code for the transfer, and 5) a description of the error or why you need additional information. We
may also ask you to select a choice of remedy (credit to your account in an amount necessary to resolve the error or altemabvely. a resend of the transfer in an amount
