MOD-83-0000496-A Republic of Iraq Questionnaire Foreusic Medicine Autopsy Report Ministry of Health Basra Health Directorate Forensic Medicine Unit Number: 2 Date: 27/05/2003 File Number: 2 To: Military Police I, signatory below, Dr. Sacod Abdul Razzaq Mahdi, have performed forensic medical autopsy on the corpse of deceased Saeed Shabram, received at 1:30 on 24/05/2003 at Basra Forensic Medicine, with autopsy request form issued by dated 24/05/2003. The results of the examination are as follows: numbered Identification of the corpse: Sex: male Height: 170cm Skin colour: wheaten Structure: medium built Head hair colour: black Eye colour: black Facial hair colour: Age: 19 years Other identifying descriptions (mole, birthmarks, malformation, organic amputation etc) especially in an unidentified body Clothes: white t-shirt - blue pants -- white trousers Post-mortem changes Rigor mortis: completed Livor mortis; rear Putrefaction: not started yet External damages: pinkish white foamy scum from the mouth. Excoriation at the lower side of the back of the left forearm measuring 1 1/2 cm X 2cm with irregular edges. Another irregular excoriation in the same area measuring 1 1/2 cm X 1/2 cm. and 5mm width. Irregular excoriation at the lower face of the right humerus measuring 18mm length Irregular excoriation at the upper right shoulder measuring 1 1/2 cm X 6mm, with an excoriation at the lower left lumbar measuring 5 mm X 6 mm - irregular. The above mentioned excoriations are most probably lively damages (not fatal). No traces of other wounds or bruises were visible in the other parts of the body during the examination. No internal bleeding was seen in the deep muscular layers. Internal Examination: Head: Hair: normal Cranial base: no trace of fracture Meninges: congested Brain: congested Facial bones: no trace of fracture Oral and dental cavity: no trace of fracture Neck: Larynx and cartilage: normal with no trace of ruptures in the cartilages of the throat Hyoid bone: no fractures in the hyoid bone Trachea: contains foamy forth. Esophagus: Thyroid gland: normal Cervical veins and nerves: normal Cervical vertebras: normal Torso: Pectoral cavity: normal Clavicle, Sternum and Rib bones: no trace of fracture Cardiac muscle and its cavity: the cardiac muscle, its cavities, valves and blood vessels look normal and the heart contain liquid blood in its cavities. Heart and main blood vessels: = Lungs and pleuras: the lungs are clearly full of water. The lungs fill most of the pectoral cavity and their surfaces are covered with patches of bleeding. Abdominal cavity: normal Peritoneum: congested Stomach: full of water mixed with digested food. Intestine: full of water Liver and gall bladder: congested and looks normal. Spleen: congested Pancreas: Kidneys, ureters, bladder and urine: congested with no obvious changes. Blood vessels: Womb (if female) The lumbar and sacral vertebras: Pelvis: no trace of fracture. The upper limbs: no trace of fracture The lower limbs: no trace of fracture Comments: Conclusion: 1. It appeared from the autopsy marks seen on the corpse of deceased Saced Shabram that the reason of death was drowning. 2. We saw a number of excoriations in the upper limbs and the back as described in the external damages section. They did not appear to have any connection to the cause of death.
