We have noticed that the card sent to you has not been activated. Your UBS Visa debit card without the chip on the front will stop working on September 25, 2015. To prevent a disruption in service, please activate and begin using your new chip card. What you need to do • Call us immediately if you did not receive your new card. • Activate your new UBS card by calling the number on the sticker. • Sign the back of your card. • Destroy your current UBS card as it will be deactivated when you activate your new card. • Contact anyone
